If you would like to make an application for admission, please click on 'Enrolment Pack'. All forms that are required for enrolment can be found here. We appreciate your assistance to complete all the required forms, in order to facilitate your application. Applications will only be processed for admission when the application and supporting documents are submitted to our Admissions Office. The following documents will need to be submitted:
- A completed application form
- Two passport-size photographs of the student
- Photocopy of birth certificate
- Photocopies of passports of student and parents (cover page, photo page, vital information page, expiration date and current visa page)
- Photocopies of KITAS for student and parents or KIMS document
- Photocopies of school reports for the previous two years.
- Photocopies of all education certificates
- Photocopies of English language test results
- Photocopies of psychological or educational evaluations/reports
- Certified translation of all documents that are not in English
- A non-refundable application fee of USD$200, paid upon submission of application (in rupiah or USD).
- Student Health Form
- Application for Bus Service Form
- Bus Permission Form
- Photo Release Form
